Affidavits, Forms, Certificates and Transcripts


The Parish Pastor prepares Baptism and Chrismation certificates; copies are emailed to the Metropolis Registry for filing (physical copies are not to be sent). To request new Baptism and Chrismation certificates, please email [email protected].

For Marriages, the Parish Pastor first fills out the Marriage Affidavit for License to Marry with the Couple. The Parish Pastor prints it, the couple signs and the Parish Pastor signs it also; the Parish Pastor then emails a scanned copy of the Affidavit to [email protected]. If a Supplemental Record Form No. 165 is needed also, the Parish Pastor also fills that out and emails it with all required and associated documents with the Affidavit. Once the Marriage is approved, a Digital Episcopal License For Marriage is emailed to the Parish Pastor.

After the completion of a Marriage, the Parish Pastor emails a scanned copy of the completed license to the [email protected] and a Certificate of Marriage is prepared. Certificates are then mailed directly to the newly-wed couple. If an email address was included when Digital Episcopal License For Marriage was completed, then a Digital Copy of the Certificate of Marriage will be emailed (this is recommened because it is a "last chance" to catch any typos or spelling errors before the physical copy is mailed out).

No "copies" or "duplicates" of certificates can be made. Instead, a transcript of data in the parish registry is prepared by the Parish Pastor, using templates below, and sent to the Metropolis Registry for the Metropolitan's verification of the Priest's signature. The completed and verified transcript is then mailed to the individual who requested it.
